WYNNEWOOD, Okla. (AP) — A southern Oklahoma refinery faces $281,000 in fines after a boiler explosion killed two workers last year.
The Occupational Safety and Health Administration announced the fines Thursday against the Wynnewood (WIN'-ee-wud) Refining Co. Employees Billy Smith and Russell Mann died Sept. 28 when a large boiler exploded at the refinery.
The OSHA report cited several violations involving the engineering and hazardous use of the boiler. The Oklahoman reports that the company had been cited for similar violations in 2008.
David Bates, the director of OSHA's Oklahoma City-area office, says it's possible the deaths could have been avoided if the company had followed OSHA standards.
CVR Energy Inc., which owns the refinery, says the company is reviewing OSHA's findings and is working to address the agency's concerns.
