LAFAYETTE, Ind. (AP) -- Indiana's workplace safety agency has cited and fined a company over the death of a worker at its Lafayette factory.
An Indiana Occupational Safety and Health Administration report says the forklift that crushed 28-year-old shipping manager Kyle Rhodes against a semitrailer had a malfunctioning parking brake and had been left unattended on an incline.
The Journal & Courier reports (http://on.jconline.com/vDm4H9 ) the agency issued nine safety citations and a $14,500 fine against Fairfield Manufacturing Co. over the May 19 death of Rhodes and an arm injury to another worker.
An agency spokesman says the fine was reduced because Fairfield is taking steps to improve safety.
Fairfield vice president Jim Mills says the company called in an outside consultant to help minimize the chance of others accidents at the 1,200-worker factory.
