The Indiana Department of Labor has hit Amazon with a safety order and proposed fines of $28,000 for safety violations that contributed to the death of a warehouse worker.
The safety order says Amazon didn't provide adequate training and safety procedures weren't followed before the Sept. 24 accident at the company's Plainfield distribution center.
Authorities say Phillip Terry, 59, of Indianapolis was fatally crushed when a forklift's lift fell on him while he was doing maintenance work on it.
The safety order, which was issued on Oct. 30, gives Amazon until Dec. 1 to correct the safety violations. The company has until Nov. 21 to respond.
Amazon said Wednesday it doesn't comment about ongoing investigations.
