A Utah man was killed over the weekend after an industrial accident at a twine manufacturing facility.
According to reports, police were called to the PolyExcel plant in Clearfield, UT around 12:10 p.m. on April 7. First responders were able to extract Jeffery Pead, 38, from the machinery however he was pronounced dead at the scene. The type of machine involved has not been reported and it is unclear at this time how Pead became trapped.
PolyExcel is a manufacturer of tree ropes, hay preserving products, industrial twines, and other products for agricultural needs.
Clearfield Police are investigating the incident and are working with the Occupational Safety and Health Administration.
