SAN ANTONIO (AP) — Federal investigators are looking into a warehouse accident in which a worker was killed when she was pinned under a 1,500-pound crate.

The San Antonio Express-News reported Friday that investigators from the Occupational Safety and Health Administration were dispatched to Con-Way Freight after 49-year-old Marissa Garcia was killed.

A forklift operator told police he had placed the seven-foot crate on the loading dock Thursday when it wobbled and pinned Garcia.

Garcia was pronounced dead at Brooke Army Medical Center shortly after midnight. The incident report said she sustained massive head trauma as a result of the accident.

Con-Way is a national trucking and shipping services firm. Company officials declined comment when contacted by the newspaper.

The OSHA probe will attempt to determine the circumstances under which the accident occurred.
