Olympic Panel Products Cited In Worker Death

Company was found in violation of safety requirements after the death of a 33-year-old worker who fell into a 7-foot pit where wood is heated in hot water.

OLYMPIA, Wash. (AP) — The death of a worker on the job has resulted in citations totaling $3,850 for a company in Shelton.
 
Dwight Midles, human resources manager for Olympic Panel Products, says the penalties ordered by the state Department of Labor and Industries will be appealed.
 
Olympic Panel was found in violation of safety requirements after the death of 33-year-old Stacey Allen of Shelton on Oct. 11. He fell into a 7-foot pit where wood is heated in water as hot as 185 degrees.
 
State inspectors wrote that an opening to the pit was left uncovered, part of a guard rail was missing and signs were inadequate. All of those problems have since been fixed.
