SPRINGFIELD, Mo. (AP) — A Springfield plant where a worker was crushed to death inside machinery has been cited for more than a dozen alleged safety violations.
The federal Occupational Safety and Health Administration said Thursday it has found one willful and 15 serious violations at Buckhorn Inc., where 31-year-old Toby Hall was killed last Oct. 7.
Officials say Hall was repairing a press when the machine was turned on. OSHA says the company failed to make sure the machine couldn't be activated while workers were inside.
OSHA has proposed $116,000 in penalties.
Buckhorn Inc. makes pallets and collapsible bulk containers.
Max Barton, a spokesman for parent company Myers Industries Inc., said the company is working with OSHA "to look at all the violations" and has requested an informal conference on the finding of a willful violation.
